Open Class Participation Consent Form

Thank you for your interest in attending Dance Stars Academy Open Class Week event. The event will take place during regular scheduled classes  (please check our Calendar page for dates) There is no cost to attend.

Please check the current  schedule on our website, and complete this consent form to confirm the attendance. Please note, all attendees must meet corresponding class age group to attend the event. Age groups are specified on the schedule.

Permission for Attendance I understand that my child will attend a Bring a Friend event, which involves dance and physical activity. I hereby waive any claim of liability on behalf of Dance Stars Academy or City of Stratford and City of Charlottetown, or it’s personnel or officers for any injuries or for efforts to obtain treatment for my child in event that my representative or I cannot be reached.
